OpenTelemetry instrumentation for Phoria Server.

Owns the Node-side OpenTelemetry setup for Phoria Server sidecars: configuration parsing from appsettings files, an OTel log adapter, NodeSDK setup for traces, metrics and logs, and an h3 span-enrichment hook.
